Your first appointment:

1. Please check your email for a message from Compass Physical Therapy LLC for a convenient link to complete the majority of information about yourself, your pain or injury, and insurance details prior to your visit. This helps insure accuracy in your record and can save you time at your appointment.

2. A second email from no-reply@webpt.com will be sent the day before your visit. This is a questionnaire required by many insurances and a useful tool to help you and your therapist determine your goals.

3. Please arrive at least 5 minutes prior to your first visit to complete any additional information including bringing your photo ID, insurance card(s), list of medications and physician’s referral if you have one.

4. If you are NOT able to complete the digital intake, please arrive at least 15 minutes prior to complete the necessary documentation. You can Download, Print and Fill-Out the Patient Medical History PDF to save time. Review the following documents ahead of time. We will provide copies for you to sign at your appointment. Reminder, this is only necessary if you are unable to complete them digitally via the welcome email.
